Not all care requires a site visit, particularly given today's technology that allows patients to communicate with their physician by:
Doc At Your Door offers a TELEHEALTH service that accommodates all of these various 'telemedicine' modalities -- whenever available and preferred by patients AND to the extent that our physician feels that an adequate assessment (and any needed treatment) can be determined and safely offered without an actual site visit. (TeleHealth services are available only to established patients who have been seen at least once in the previous 12 months.)
One of the biggest challenges patients face in our current health care system is the changing of care settings (home to hospital, hospital to rehab, etc.)
In better days gone by, a single physician would care for a patient wherever they needed that care -- in the office, at home, at the hospital, at rehab, at the nursing home, wherever. All of this care by the same physician was coordinated, consistent, and efficient.
Unfortunately, this is no longer the case . . . each care setting now prefers to provide its own dedicated physician or physician team: there is one physician for office care, one for ER care, one for ICU care, one for general hospital care, one for rehab care, one for nursing home care, and on and on.
Sadly, having different physicians at each level of care often interferes with the overall quality of that care, partly because physicians often do a poor job of communicating and sharing information with each other when care settings change. Each different physician must 'start from scratch' with patients and is not necessarily fully informed about prior conditions, tests, treatments, etc. Care becomes fragmented, inefficient, and even detrimental at times. At a minimum, it is extremely frustrating to patients and families.
Doc At Your Door offers a solution, providing critical TRANSITIONAL CARE whenever needed, ensuring that all providers caring for a patient are essentially on the same page! Patient care can once again be fully coordinated and managed from any setting to another:
Our physician (with 30 years of experience doing the same) can meet with outgoing and incoming providers, ensure full information and record sharing, compare treatment plans between settings to avoid omissions and redundancies, follow-up on pending labs and tests, reconcile medications between settings, ensure needed equipment and supplies are available at a new setting, ensure required follow-ups are arranged, and otherwise assist as necessary to ensure a smooth, efficient, and safe transition.
As our health care system grows more and more complex each year, it can now be absolutely overwhelming for an individual, or their family, to figure out what care is needed, from whom, when and where, in what order, and at what cost. Figuring out co-pays, co-insurance, deductibles, formularies, Tier 1s, 2s, & 3s, pre-authorizations, denials, appeals, network participation, and secondary insurance; understanding observations vs admissions, 3-night stay rules, and advanced beneficiary notices; choosing the best specialists, imaging centers, hospitals, extended care facilities, and home health agencies; arranging follow-up appointments, labs tests, x-rays, and procedures; ensuring new medications are safe to take with old medications; arranging transportation, mail order medications, equipment, supplies, O2, home health nurses; and so much more . . . it can seem impossible at times!
